If you're a new client, please review the following document and have your questions ready for your first therapy session. I will provide a copy for signatures.  

• Client Handbook   (practice info / informed consent with signatures, fee schedule, Client Intake, Authorization to Disclose form, etc. 

If you would like me to coordinate care with another provider (your psychiatrist, primary care physician, etc.) we must complete the above form for disclosure.
